Two of the major challenges of managing a post-acute care program include an effective discharge system and identifying the post-acute providers who will work with your organization
to ascertain the best patient outcomes.

In order to ensure effective placement of patients to the appropriate level of care and to reduce the workload of the hospital case management team, PAC Plus will hire and implement a team of individuals with expertise in each level of care to coordinate the discharge plan. This team will work with the patients, post-acute providers, your hospital case managment and other related parties to ensure a smooth transition from the acute care setting to post-acute organization. All of this is done with careful documentation in PAC Plus software and continued communication with hospital case management.

Our PAC Plus team will help your organization identify the right post-acute care providers and
monitor their outcomes on a weekly and monthly basis. This includes monthly consortium meetings with all post-acute providers within the network and weekly on-site visits to review
patient progress and outcomes. Other topics included during our post-acute provider meetings include average length of stay, readmission rates, and cost containment.
